Form used by members of the Judiciary and certain judicial appointees to disclose particularly relevant information concerning their financial affairs.
The financial disclosure statement can now be filed in paper form or online:

NOTE TO FILERS: The instructions have been modified to permit filers to disclose only the county and state of ANY real properties that they own.
The online financial disclosure statement which has been developed for use by Active and Recalled Judges, Orphans' Court Judges, Magistrates, District Court Commissioners, Court Auditors, and Examiners is now available at the link below:
Once at the site, click on “Set up Account” on the first screen, enter the required information (first name, middle initial, last name, etc.), then select “Register”. You may then proceed with creating your statement. The system will also send you an email with your login information to retain for your records.
